Getting an MV-15 form from NY DMV

Is there anyone out there that can help us get a completed MV-15 form out of the DMV in Albany. We submitted the form 18 weeks ago and crickets from the DMV who will not respond to emails or voice mail. I am trying to import the car to Canada from NY and need to have this stupid piece of paper completed before I can go any further
HELP!
